Only 71 psilocybin treatment centers in the US hold a state license

Three different things get sold under one phrase. 71 centers hold a live state license to administer psilocybin: 24 in Oregon with an OHA license on record and 47 approved in Colorado, with 6 more Colorado applications pending. Roughly 1,942 US clinics treat with ketamine, which is a different drug under a different law. Everything else is a retreat abroad or a research study. Here's how to tell which one you're looking at.

The license is the whole distinction

A licensed center answers to a regulator that can shut it down. In Oregon that's the Oregon Health Authority under ORS 475A; in Colorado it's the Natural Medicine Division, and every healing center carries a numbered license like NMHC-00055 that you can look up. We print those numbers on the records because a center advertising "psilocybin therapy" without one is doing something else, whatever the homepage says.

Ketamine clinics are the confusing case. They're real medicine, prescribable in all 50 states, and often excellent. They are also not psilocybin, and a fair number market themselves in language that blurs the two. Check the drugs listed on any record before you call.

What we'd tell a friend

If the goal is psilocybin specifically and you can travel, Colorado's license register is the cleanest starting point in the country, because approved and pending status is public per center. If the goal is relief from depression and you're flexible about the molecule, a ketamine clinic will see you in a week and the evidence base is deeper. If money is the constraint, a trial is free and you should start reading the enrollment process now, since the screening queue is the long pole.
