Two US states run licensed psilocybin programs. Registered clinical trials are legal in all fifty. A short list of countries never scheduled the compound at all. That's the whole map, and every claim on this page is backed by one of the 2,473 treatment sites and 1,631 registered studies in the index.

Psilocybin remains federal Schedule I, and the FDA pipeline is where the medical story is moving: breakthrough-therapy designations for depression and active phase 2 and 3 programs. A registered trial is the one fully legal route in all fifty states, treatment is run by medical teams, and participation costs nothing. We count 314 US sites recruiting right now, 576 worldwide.
